For lost or stale dated residuals checks, please complete and submit a Stop Payment/Reissue Request Form or contact Stop Payment/Reissue Department at 323-549-6567. Please note: SAG-AFTRA's policy regarding stop payment on checks is 30 days from the original mail date.
