This is a form to be signed by the authorized person on behalf of the company. It binds the company to the SAG-Producers Pension and Health Plans.

This is a list of performers that the producer intends to cast in the production.

The appropriate company representative completes this form when a business entity is producing the SAG-signatory new media production.

To begin the SAG-AFTRA New Media signatory process, a producer or their representative should complete this form and submit it along with the Pre-Production Cast List form.
